How uses Asteroid to complete 30,000+ care coordination tasks per month

Thyme Care is a cancer care navigation company that helps people with cancer make sense of a healthcare system that’s often confusing and overwhelming. A multi-disciplinary Care Team including oncologists, physicians, social workers, nurses and care partners coordinate everything from transportation and financial assistance to nutrition, emotional support, and clinical guidance alongside their medical treatment. It operates virtually across all 50 states, partnering with health plans and primary care groups to make this support available to their members at no direct cost.

Every touchpoint generates documentation that has to reach everyone treating the patient: their oncologist, their primary care doctor, Thyme Care’s own Care Team, and every other provider involved. Much of that communication still relies on fax, while some can be accomplished through different Electronic Health Record (EHR) uploads.

Thyme Care’s team had already built a reliable process for generating this documentation and sending it where it needed to go. The challenge became volume: as Thyme Care’s patient population grew with new and expanded customers, the coordination workload grew with it — faster than any team could scale by hand.

What they built: transitions of care coordination, automated end to end

Take one core workflow: when a patient is discharged from a hospital, Thyme Care’s team is notified and a nurse outreaches the patient to support their transition. Nurses document that encounter in Thyme Care’s own care management tool. Within one day, Asteroid picks up that information and automatically routes it via the proper channel to its destination — whether via e-fax or distinct EHR upload.

Time-sensitive documentation that used to sit in a queue for several days now reaches a patient’s Primary Care Physician (PCP) and Oncologist promptly, often ahead of the patient’s post-discharge visit, in time to actually inform the next treatment decision.

This is one of more than ten daily workflows Thyme Care now runs on Asteroid, spanning all sorts of care coordination work and task prioritization to keep the team focused daily on the most important needs.

This was all built and maintained by one member of Thyme Care’s product team using Astro, the assistant built into the Asteroid platform. Astro can reference the full context of Thyme Care’s existing workflows and executions, which makes building each incremental new workflow even faster and more reliable.
